| AUTOMOBILE CLAIM PROCEDURE |

1. If you had an accident, you must report your claim. Report your claim through facsimile or by phone (at least 3 x 24 hour after the accident occurred)
Please inform us the following information:
- Your Insurance Policy Number
- The Name stated in the Insurance Policy
2. Fill in our Automobile Claim Form and providing information regarding:
- Automobile Insurance Policy and Vehicle Information.
- The Accident's Chronologies
- The Accident's Sketches
3. Please provide the following documents:
- Copy of Vehicle Registration (STNK)
- Copy of Driver's Driving License (SIM)
- Copy of The Automobile Insurance Policy
4. Loss Survey shall be conducted by PT. Asuransi Tokio Marine Indonesia's Automobile Claim Officer. |
